Dr. Pineda is a contracted provider with Cigna and Anthem. If you have a different insurance plan, you would have to pay Dr. Pineda's standard fees and submit a claim to your insurance.
In order to confirm your appointment, we will need to verify your insurance information, unless you do not wish to submit a claim and will be paying out of pocket.
You can submit a copy (front and back) of your insurance card via email to dreduardopineda@gmail.com or fax at (909) 727-8012.

Once we confirm your appointment, please review the following documents. You may also print and complete them in advance and bring them with you to your appointment:
The Office Policies and Procedures,
The Acknowledgement of Receipt of the Notice of Privacy Practices, and
Please completeThe Authorization for Release of Health Information form, if you wish for Dr. Pineda to be able to communicate with your primary care provider, therapist, family member(s), etc.
